It's a normal day and Conan is home sick with the flu. He gets a call from Ai about a possible new antidote for APTX 4869. He rushes over to Dr. Agasa's to try it. After taking it, the effect is near instant and he passes out. Upon waking, he is back in his old body. He rushes to Teitan High School to surprise everyone. However, nothing is right. He doesn't recognize his classmates and everything seems different. He comes across a Detective Club that he doesn't remember being there before he was shrunk. Inside, he meets Ayumi, Genta, and Mitsuhiko, only they have aged 10 years! Just what is going on?

At the Mouri Detective Agency, it's a slow morning. The birds are chirping and there isn't much else exciting going on. Conan has come down with a cold. Ran is checking on him before going to school with Sonoko. Outside his room, Conan hears Sonoko ask Ran about Shinichi. As they walk out, Sonoko says that Ran should just dump him and start to date Dr. Araide. Conan rushes to the balcony to catch more of their conversation. He imagines himself walking behind them wondering if he will ever change back before going back inside and laying in bed. He blames his negative thoughts on his cold before falling asleep.

While sleeping, Conan's cell phone rings. It is Ai calling about his cold. She mentions how it is ironic since she just created a new prototype of the antidote. He pops up and asks Ai where to meet. He immediately changes clothes and rushes to Agasa's house. After Conan leaves the building, Kogoro comes out of his office to make some soup for Conan, only to find him gone when he reaches his room.
Conan jogs past his old house before doubling back and getting some of his old clothes for when he changes back. Ai is inside drinking tea when Conan bursts in carrying his old school uniform. He explains that he was planning to suddenly show up at school and surprise everyone. Ai warns him of it's foolish nature to which he ignores. She takes his temperature (38.7º C) to make sure the new antidote will be effective. After another warning, she explains that this version has a quick reaction and will change him immediately upon taking the pill. He goes into the bathroom to take the pill. The effect is instant and he is back in his old body.
He comes out of the bathroom to an empty house. Calling for her, Ai is nowhere to be found. It's 12:30pm and he still plans to go to the school and greet all his old friends. He walks by his house noticing that the tree in his front yard is considerably taller than it use to be. He chalks it up to being Conan so long and the different angle from being so short. He continues into town where he discovers buildings that previously weren't there and a recently renovated apartment complex.
He arrives at the school and spots one of his teaches and he has aged so much since he shrunk. Conan goes to his locker only to discover it's not the right one. He searches all the nearby lockers but can't seem to find his. He arrives at class 2-B only to be disappointed no one is surprised to see him. Looking around the room, he doesn't recognize any of the kids in the class. He decides to look for Ran in the Karate Club as he is normally there during the lunch break.
He comes across a room marked "Detective's Club" and wondered if it was there before he became Conan. He goes in to be greeted by a group of 3 students. Conan doesn't recognize them at first but eventually sees that they are the Detective Boys, Ayumi, Genta, and Mitsuhiko, as high school students. He is cautiously wondering what is going on and why they are so much older and why he doesn't remember anything that has happened in this apparent 10 year gap.
